Process of Creating an Individual Service Plan

To facilitate the development of an Individual Support Plan (ISP) for each customer and to co-ordinate implementation, monitoring and subsequent revisions of each plan is the core process of BRS, Inc. We are committed to the philosophy of assisting to empower each customer to advance toward an enhanced quality of life as defined by that particular individual. Our staff works from a team approach and we are geared towards co-operating for the best interests of our customers both within BRS, Inc. and with other individuals and agencies as well.

Individual Support Plans

A primary working principle is to maximize community involvement and natural interpersonal support systems, whenever possible, in the interest of both our customers and the larger community. Individual Support Plans may include, but are not necessarily limited to, some or all of the following issues:

  • Housing and Residential Needs
  • Training, Employment and Employment Support
  • Transportation
  • Recreation
  • Health, Medical Treatment and Medication
  • Habilitation and Personal Assistance Services
  • Behavior Support Plans
  • Individual Crisis Plans

Eligibility for Services

Individuals requesting services provided by BRS, Inc. will have been determined eligible for services by DMRS or will be required to complete and submit the Initial Service Request to the regional DMRS office.

BRS, Inc. will accept referrals for individuals with disabilities to existing services consistent with eligibility guidelines, programmatic needs, and waiting list limitations. Referrals will be provided by contracted agencies that provide referral and case management services.

BRS, Inc. is committed to assisting other agencies within our service delivery system in identifying individuals in need of services.

Family Support Important to Community Living

Tennesseans with severe disabilities may be eligible for financial assistance through the Family Support Program, contracted through the Division of Mental Retardation Services. The program provides allocations of funds for services ranging from respite/personal assistant to equipment/ home modifications. People with severe disabilities may have needs that insurance does not cover. The Family Support Program may be the answer to these needs. The program is funded statewide through the DMRS and each county has an agency that oversees local funding. Buffalo River Services, Inc. coordinates funding for Maury, Lawrence, Giles, Hickman, Perry, Lewis and Wayne counties in Middle Tennessee. Local councils share by advising agencies, providing oversight, and making recommendations on funding needs and priorities for services in their communities. District and State councils, composed of a majority membership of individuals with disabilities or their families, participate in developing policies and procedures to help carry out the program. There is a process for determining eligibility and selection of families or individuals to receive services. The primary focus of the Family Support program is to provide services to families whose family member:

  • Was born with severe disabilities, or acquired them in childhood
  • Has been severely disabled by injury or trauma, such as brain injury or spinal cord injury
  • Has neurological and/or neuromuscular disorders, such as ALS, MD, MS

Factors may include:

  • Family needs, including services currently available
  • Immediacy of the need-crisis or emergency
  • Severity of the family problems
  • Time awaiting services
  • If funding is available
